All data boxes must be filled in. If a question is non-applicable to either borrower please enter "n/a" in the appropriate data box. If a question is applicable to you, but you do not have the answer, please enter "unk" (meaning "unknown") in the appropriate data box.
Manner in which Title will be Held
- S&S:
- If you will hold title as an unmarried man or woman, or if a spouse will not be
holding title with the applicant.
- JTw/ROS:
- Joint Tenants with Rights of Survivorship. Please confer with your
local Title Agency or counsel to determine if this form of ownership is
suitable.
- CPw/ROS:
- Community Property with Rights of Survivorship. Please confer with
your local Title Agency or counsel to determine if this form of ownership is
suitable.
Amortization Type of Mortgage Desired
- Fixed:
- The interest rate will never change during the life of the mortgage.
- Interest Only:
- The interest rate never changes and the note does not have any
amortization schedule. Generally such notes have a balloon payment
provision.
- ARM:
- Adjustable Rate Mortgage: the Note will have a provision that will call for
the interest rate to adjust on a defined periodic basis and based on a pre-
determined index, margin, and adjustment limitation.
Type of Mortgage Applied For
- FHA:
- Federal Housing Administration. Federal Government insures the mortgage.
There are restrictions in securing FHA financing including that such financing is generally for first time home buyers and there are loan limits for each region throughout the US.
- VA:
- Veterans Administration. Federal Government guarantees the mortgage.
The borrower must be veteran eligible to apply. Such program is for purchase money financing only (no refinancing).
- Conv:
- Conventional. Every other type of financing available except for a privately
funded mortgage. Generally Conventional financing comes from institutional lenders utilizing either government backed funding sources or their own deposits to fund their mortgages. There are generally substantial financing options for a borrower to choose from to secure their applied for financing based on their respective qualifying factors.
- Priv:
- Private. When conventional financing options are not available to a
Borrower. Selecting for a private funding source does not guarantee approval. A borrower must qualify for what ever programs are provided by the private funding sources offered.
Detail your Compensation
Define all other forms of income you may presently receive, including the amount of that income per year or per month (e.g., bonuses, overtime, rental profit from investment property, stock dividends, interest income from security or notes, etc.)
Define Other Miscellaneous Income
Enter the amount of your monthly base income from any additional income such as overtime, rental, dividend, and any other income. Please enter the dollar amount and provide the detail of your miscellaneous income at the bottom of this application in the general information text box.

- Personal Residence.
- SH:
- Second Home. This is not to be considered a rental property. Family
Members living in the home can be considered a second home.
- IP:
- Investment Property.

- S:
- Separate. Either as a single person or as a married person but as sole and
separate.
- SP:
- With a spouse.
- O:
- Other. Examples could be as a Tenant in Common with a partner, in a
Corporate entity, in a Trust, as an LLC, or some other form.
Ethnicity
Hispanic or Latino. A person of Cuban, Mexican, Puerto Rican, South or Central American, or other Spanish culture or origin, regardless of race. The term, "Spanish origin," can be used in addition to "Hispanic or Latino."

- American Indian or Alaskan Native:
- A person having origins in any of the original peoples of North and South America (including Central America), and who maintains tribal affiliation or community attachment.
- Asian:
- A person having origins in any of the original peoples of the Far East,
